Dac\u0103 aceste procese nu func\u021bioneaz\u0103 corect, homocisteina se acumuleaz\u0103 \u00een s\u00e2nge, determin\u00e2nd o stare numit\u0103 hiperhomocisteinemie.<\/p>\n<\/p>\n<p>Nivelul crescut al homocisteinei este asociat cu un risc mai mare de boli cardiovasculare (infarct miocardic, accident vascular cerebral), tulbur\u0103ri de coagulare, tromboze venoase \u0219i avorturi spontane recurente. De asemenea, homocisteina contribuie la deteriorarea vaselor de s\u00e2nge prin favorizarea inflama\u021biei \u0219i a stresului oxidativ.<\/p>\n<\/p>\n<p>Analiza homocisteinei este util\u0103 at\u00e2t pentru identificarea unui risc cardiovascular ascuns, c\u00e2t \u0219i pentru investigarea posibilelor deficien\u021be nutri\u021bionale (vitamina B12, acid folic), tulbur\u0103ri genetice (cum ar fi muta\u021biile genei MTHFR) sau afec\u021biuni rare, precum homocistinuria \u2013 o boal\u0103 ereditar\u0103 caracterizat\u0103 prin acumularea excesiv\u0103 de homocistein\u0103.<\/p>\n<\/p>\n<p><strong style=\"color: #ff9900\">1. Indica\u021bii testare<\/strong><\/p>\n<ul>\n<li>Evaluarea riscului cardiovascular, \u00een special la pacien\u021bi f\u0103r\u0103 factori de risc tradi\u021bionali.<\/li>\n<li>Investiga\u021bia cauzelor trombozelor venoase profunde, a emboliei pulmonare sau a altor episoade de coagulare anormal\u0103.<\/li>\n<li>Explorarea cauzelor avorturilor spontane recurente.<\/li>\n<li>Suspiciunea de deficit de vitamina B12 sau acid folic, mai ales c\u00e2nd hemoleucograma este modificat\u0103 sau pacientul prezint\u0103 semne neurologice.<\/li>\n<li>Screening pentru homocistinurie, mai ales la copii sau adul\u021bi tineri cu fenotip specific ori evenimente trombotice inexplicabile.<\/li>\n<li>Evaluarea pacien\u021bilor cu muta\u021bii ale genei MTHFR (C677T, A1298C), \u00een special \u00een contextul trombofiliilor ereditare.<\/li>\n<\/ul>\n<p><strong style=\"color: #ff9900\">2. Preg\u0103tire pacient \u0219i mod de recoltare<\/strong><\/p>\n<ul>\n<li>Se recomand\u0103 repaus alimentar de 8\u201312 ore \u00eenaintea recolt\u0103rii (pe nem\u00e2ncate, <em>\u00e0 jeun<\/em>).<\/li>\n<li>Pacientul trebuie s\u0103 informeze medicul cu privire la suplimentarea sau tratamentul cu vitamine B (B6, B12, acid folic), care pot influen\u021ba valorile ob\u021binute.<\/li>\n<\/ul>\n<p><strong style=\"color: #ff9900\">3. Interpretare rezultate<\/strong><\/p>\n<p><strong>Valori crescute:<\/strong><\/p>\n<ul>\n<li>Deficit de vitamina B12, acid folic sau vitamina B6.<\/li>\n<li>Boli renale cronice \u2013 homocisteina este eliminat\u0103 prin rinichi.<\/li>\n<li>Afec\u021biuni genetice, cum ar fi muta\u021biile genei MTHFR.<\/li>\n<li>Homocistinurie \u2013 boal\u0103 metabolic\u0103 rar\u0103 ereditar\u0103.<\/li>\n<li>Factori de stil de via\u021b\u0103 \u2013 fumat, diet\u0103 dezechilibrat\u0103, alcoolism cronic.<\/li>\n<li>Asociere cu risc crescut de: Boal\u0103 cardiovascular\u0103: ateroscleroz\u0103, infarct, accident vascular cerebral.<\/li>\n<li>Tromboze venoase profunde \u0219i embolii pulmonare.<\/li>\n<li>Complica\u021bii asociate sarcinii: avorturi recurente, preeclampsie.<\/li>\n<\/ul>\n<p><strong>Valori sc\u0103zute:<\/strong><\/p>\n<p> Nu au semnifica\u021bie clinic\u0103 proprie, dar pot ap\u0103rea la pacien\u021bii afla\u021bi sub tratament cu suplimente de vitamina B sau acid folic.<\/p>\n<\/p>\n<p><strong style=\"color: #ff9900\">6. Factori care pot influen\u021ba rezultatul<\/strong><\/p>\n<ul>\n<li>Dieta s\u0103rac\u0103 \u00een vitamine B12, B6, acid folic.<\/li>\n<li>Medicamente: metotrexat, fenitoin\u0103, anticoncep\u021bionale orale, levodopa, teofilin\u0103.<\/li>\n<li>Proceduri medicale: anumite investiga\u021bii imagistice cu substan\u021b\u0103 de contrast pot modifica nivelul homocisteinei.<\/li>\n<li>Afec\u021biuni cronice: boal\u0103 renal\u0103 cronic\u0103, hipotiroidism, afec\u021biuni hepatice.<\/li>\n<li>Defecte genetice ale enzimelor implicate \u00een metabolizarea homocisteinei (ex. deficien\u021ba MTHFR).<\/li>\n<\/ul>\n<p><strong style=\"color: #ff9900\">7.
