Descriere general\u0103<\/strong><\/p>\n<p>Glanda tiroid\u0103 este o gland\u0103 de dimensiuni mici, \u00een form\u0103 de fluture, situat\u0103 \u00een partea anterioar\u0103 a g\u00e2tului. Este o component\u0103 esen\u021bial\u0103 a sistemului endocrin, responsabil\u0103 de producerea hormonilor tiroidieni, care joac\u0103 un rol cheie \u00een reglarea metabolismului, cre\u0219terea, dezvoltarea \u0219i func\u021bionarea optim\u0103 a \u00eentregului organism. Hormonii tiroidieni, tiroxina (T4) \u0219i triiodotironina (T3) sunt produ\u0219i sub controlul TSH (hormonul de stimulare tiroidian\u0103) secretat de hipofiz\u0103.<\/p>\n<\/p>\n<p>Tiroxina (T4) este produsul principal de secrea\u021bie al glandei tiroide \u0219i este considerat un prohormon, fiind ulterior convertit \u00een T3 la nivelul \u021besuturile periferice. \u00cen s\u00e2nge, cea mai mare parte a T4 este legat\u0103 de proteinele plasmatice de transport (TBG- thyroid binding globulin, albumin\u0103) \u0219i doar o cantitate mic\u0103 circul\u0103 liber\u0103. <\/p>\n<p>FT4 sau tiroxina liber\u0103 reprezint\u0103 frac\u021biunea liber\u0103, biologic activ\u0103 a tiroxinei, responsabil\u0103 de efectele metabolice.<\/p>\n<\/p>\n<p>Printre principalele roluri ale hormonilor tiroidieni se num\u0103r\u0103: <\/p>\n<ol>\n<li>Reglarea metabolismului: hormonii tiroidieni cresc rata metabolic\u0103 bazal\u0103 \u0219i consumul de oxigen, promov\u00e2nd arderea glucidelor \u0219i lipidelor pentru energie. De asemenea, ajut\u0103 la reglarea \u0219i men\u021binerea temperaturii corporale.<\/li>\n<li>Func\u021bionarea adecvat\u0103 a sistemului nervos: sunt vitali pentru cre\u0219terea \u0219i dezvoltarea normal\u0103 a copiilor, dar au \u0219i un impact semnificativ asupra proceselor cognitive \u0219i emo\u021bionale la adul\u021bi.<\/li>\n<li>Cresc frecven\u021ba \u0219i for\u021ba contrac\u021biilor cardiace, influen\u021b\u00e2nd astfel debitul cardiac \u0219i circula\u021bia sangvin\u0103. <\/li>\n<li>Asigur\u0103 mineralizarea corect\u0103 a oaselor \u0219i men\u021bin masa \u0219i for\u021ba muscular\u0103.<\/li>\n<li>Faciliteaz\u0103 procesul de digestie, prin promovarea motilit\u0103\u021bii \u0219i secre\u021biei digestive. <\/li>\n<li>Men\u021bin echilibrul hormonal necesar pentru fertilitate \u0219i s\u0103n\u0103tatea reproductiv\u0103.<\/li>\n<\/ol>\n<p>FT4 este un test de baz\u0103 pentru evaluarea func\u021biei tiroidiene \u0219i diagnosticul dezechilibrelor hormonale. Interpretarea sa trebuie realizat\u0103 \u00eempreun\u0103 cu TSH \u0219i, la nevoie, cu T3 \u0219i al\u021bi markeri specifici.<\/p>\n<\/p>\n<p><strong>2. Indica\u021bii testare<\/strong><\/p>\n<ul>\n<li>Evaluarea de rutin\u0103 a func\u021biei tiroidiene.<\/li>\n<li>Investigarea pacien\u021bilor cu semne\/ simptome sugestive pentru disfunc\u021bie tiroidian\u0103: oboseal\u0103 persistent\u0103, tulbur\u0103ri de tranzit intestinal, modific\u0103ri ale greut\u0103\u021bii, palpita\u021bii, intoleran\u021b\u0103 la frig sau c\u0103ldur\u0103, tulbur\u0103ri de cre\u0219tere la copii.<\/li>\n<li>Diagnosticul \u0219i monitorizarea tratamentului disfunc\u021biilor tiroidiene: hipotiroidism sau hipertiroidism (\u00eempreun\u0103 cu TSH).<\/li>\n<li>Evaluarea nodulilor tiroidieni.<\/li>\n<li>Screeningul disfunc\u021biilor tiroidiene \u00een sarcin\u0103.<\/li>\n<\/ul>\n<p><strong>3. Preg\u0103tire pacient<\/strong><\/p>\n<ul>\n<li>Se recomand\u0103 recoltarea diminea\u021ba a jeun (pe nem\u00e2ncate).<\/li>\n<li>La pacien\u021bii \u00een tratament cu levotiroxin\u0103, recoltarea se face \u00eenainte de administrarea dozei zilnice.<\/li>\n<li>Se recomand\u0103 informarea medicului despre tratamentele actuale.<\/li>\n<\/ul>\n<p><strong>4. Tipul probei recoltate: <\/strong>S\u00e2nge venos<\/p>\n<\/p>\n<p><strong>5. Valori de referin\u021b\u0103 <\/strong><\/p>\n<p>Valorile de referin\u021b\u0103 pot varia \u00een func\u021bie de laborator \u0219i metoda de dozare.<\/p>\n<p>Valorile normale ale FT4 variaz\u0103 \u00een func\u021bie de v\u00e2rst\u0103 \u0219i starea fiziologic\u0103 (sarcin\u0103).<\/p>\n<ul>\n<li>Adul\u021bi: 0.8-1.8 ng\/dL (10-23 pmol\/L)<\/li>\n<\/ul>\n<p><strong>6. Interpretare rezultate<\/strong><\/p>\n<p>Rezultatele se interpreteaz\u0103 \u00een context clinic, al\u0103turi de valorile TSH \u0219i alte investiga\u021bii.<\/p>\n<\/p>\n<p><strong>Valori crescute<\/strong><\/p>\n<p>Pot inidica hipertiroidism (exces de hormoni tiroidieni), care poate s\u0103 apar\u0103 \u00een:  <\/p>\n<ul>\n<li>Boala Basedow-Graves (boal\u0103 tiroidian\u0103 autoimun\u0103 caracterizat\u0103 prin produc\u021bie crescut\u0103 de hormoni tiroidieni la nivelul tiroidei)<\/li>\n<li>Nodul tiroidian toxic sau gu\u0219\u0103 polinodular\u0103 toxic\u0103<\/li>\n<li>Tiroidit\u0103 subacut\u0103<\/li>\n<li>Administrare excesiv\u0103 de hormoni tiroidieni<\/li>\n<li>Tulbur\u0103ri ale glandei hipofize<\/li>\n<\/ul>\n<p>\u00cen sarcin\u0103 (trimestrul I), valori u\u0219or crescute pot fi normale.<\/p>\n<\/p>\n<p><strong>Valori sc\u0103zute<\/strong><\/p>\n<p>Pot inidica hipotiroidism (produc\u021bie insuficient\u0103 de hormoni tiroidieni), care poate s\u0103 apar\u0103 \u00een:<\/p>\n<ul>\n<li>Tiroidita Hashimoto<\/li>\n<li>Deficit de iod<\/li>\n<li>Postchirurgical sau post-terapie cu iod radioactiv<\/li>\n<li>Afec\u021biuni hipofizare<\/li>\n<li>Tratament necorespunz\u0103tor cu hormoni tiroidieni sau antitiroidiene de sintez\u0103.<\/li>\n<\/ul>\n<p><strong>7. Factori care pot influen\u021ba rezultatul<\/strong><\/p>\n<ul>\n<li>Medicamente: estrogeni, anticoncep\u021bionale orale, amiodaron\u0103, corticosteroizi, anticonvulsicante \u2013 pot determina cre\u0219teri sau sc\u0103deri la FT4.<\/li>\n<li>Afec\u021biuni sistemice severe \u2013 pot produce modific\u0103ri ale hormonilor tiroidieni (valori sc\u0103zute) f\u0103r\u0103 s\u0103 existe o boal\u0103 tiroidian\u0103 real\u0103 (euthyroid sick syndrome).<\/li>\n<li>Suplimentele cu biotin\u0103 \u00een doze mari \u2013 pot interfera cu metoda de dozare.<\/li>\n<\/ul>\n<p><strong>8. Analize complementare<\/strong><\/p>\n<ul>\n<li>TSH \u2013 pentru evaluarea func\u021biei tiroidiene \u0219i confirmarea dezechilibrelor hormonale.<\/li>\n<li>FT3 (triiodotironin\u0103 liber\u0103), T3 total \u2013 pentru completarea diagnosticului, mai ales \u00een hipertiroidism.<\/li>\n<li>Anticorpi antitiroidieni (anti-TPO, anti-tiroglobulina, TRAb) \u2013 \u00een suspiciunea de boal\u0103 autoimun\u0103.<\/li>\n<li>Ecografie tiroidian\u0103 \u2013 pentru evaluarea structurii glandei tiroide.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>FT4 (tiroxina liber\u0103) m\u0103soar\u0103 frac\u021biunea liber\u0103, biologic activ\u0103 a hormonului tiroidian T4.
